📝 Make a Referral to Adult Social Care

Anyone can refer someone for social care support – including the person themselves, a carer, a professional, or a concerned friend.

🔍 When to Make a Referral

You should refer someone if they:

  • Are struggling with daily tasks like washing, cooking or getting dressed
  • May be at risk of abuse, neglect or self-neglect
  • Have health conditions affecting their independence
  • Are unpaid carers in need of support

🛠️ What You’ll Need

  • Full name, address & contact info
  • Date of birth & GP details
  • Health or disability information
  • Daily challenges they face
  • Consent (unless it’s a safeguarding concern)

📬 How to Make a Referral

  1. Search your council’s website for “adult social care referral”
  2. Use their online form or call the Adult Social Care Team
  3. If urgent,  please call and then you'll be redirected to the appropriate team

🛡️ Safeguarding Concerns?

If you suspect abuse or neglect:

  • Don’t wait for consent
  • Call the council or raise a concern anonymously

📄 What Happens Next?

  • The council will decide if a Care Act assessment is needed
  • They may signpost you to other support or services 
  • It is likely you'll be placed on a waiting list whilst they allocate you a worker 
  • They’ll contact the person and/or the referrer directly
  • A full assessment and  support plan will take place - depending on the workers decision this may take place over the phone, home visit or a meet up in a public space.

💬 Want an independent assessment?

Sometimes you will be placed on a significant and timely waiting list, or maybe you didn't agree  with the original assessment completed by the local authority. Then your next option could be to consider getting an independent social worker. Please note: Requesting an independent social worker will come at a cost and will still require you to pay for the support suggested or have the local authority agree the level of funding you feel you require.
